The Collaborative Care Model initiative is a selective opportunity for orthopedic surgeons and practices to help shape and evaluate a refined recovery model— combining concierge-level orthopedic nursing with meaningful recovery data. Each patient is supported by one dedicated orthopedic nurse who provides ongoing assessment, education, guidance, and recovery coordination while keeping your practice informed of meaningful recovery developments throughout the postoperative period.

Remote Therapeutic Monitoring (RTM)

Luxe Ortho incorporates advanced Remote Therapeutic Monitoring (RTM) to provide objective recovery data throughout the postoperative period for several Experience tier packages and also available in the Curated menu offered . Remote Therapeutic Monitoring (RTM) combines objective recovery data with experienced orthopedic nursing. While RTM continuously measures recovery metrics such as swelling, temperature, pain reporting, range of motion, and patient-reported outcomes, your patient’s orthopedic nurse interprets those findings in the context of the patient’s clinical recovery. Technology provides the data. The nurse provides the clinical judgment, education, and communication that help transform information into meaningful patient care.

Objective monitoring allows concerns to be identified earlier while providing meaningful recovery information that can be shared with the surgical team.

Getting Started

1. Identify an appropriate patient— patients undergoing elective orthopedic surgery who are seeking a dedicated, RN-led recovery experience beyond what standard home health provides

2. Ideally, introduce Luxe Ortho at the time surgery is scheduled— early introduction allows time for preoperative planning, enrollment, home safety evaluation, and RTM enrollment, set up and education

3. With patient authorization, we coordinate recovery services directly with the patient— care begins with an Extended Comprehensive visit for preoperative planning, followed by the day of surgery, post-op day one and others as chosen. Additional Focused Recovery visits based on the client’s selected Experience package. The scope and cadence of visits are determined by the clients chosen experience package, selected during the initial discovery call

4. We maintain communication with your office throughout the recovery journey, sharing data, trends and flagging concerns as they emerge

5. All coordination adheres to HIPAA guidelines, with documentation available to support your practices clinical record, as requested
